The information provided in this form accounts for the use of funds and demonstrates funding has been used for the purpose it was provided under the Funding Agreement.

Failure to supply an Acquittal Form will result in applicants being ineligible for sponsorship in subsequent years until a completed Acquittal Form is provided.

Westernport Water reserves the right to terminate the Sponsorship Agreement or seek repayment of any funding if the applicant has not completed the Acquittal Form or met the conditions of the Funding Agreement.
